- Enter in the type of insurance it is, the name, the plan number, group name, etc..
- You can leave any spot blank if you don't wish to share certain information.
- You can make this your current or previous insurance plan and add notes, like a $40.00 co-payment.
- You can also add other insurance profiles like prescription plan, dental, and vision.
